Patient Advocate Service

Being informed and understanding what to expect from various healthcare services allows you the benefit of knowing that you are receiving appropriate services to meet your needs and helps create optimal outcomes.  Whether you are receiving assistance from private duty caregivers in your home or progressively increased care attention through Home Health services or a healthcare facility, Alastar will be there to provide supervision, guidance and answers to your questions.

Alastar also works closely with medical professionals, including Physicians and Case Managers, to provide transitional care post-hospitalization with the goal of avoiding unnecessary costly and possibly traumatic re-admissions to acute care settings.

Being informed and having a realistic expectation as to what may occur during an admission to a Skilled Nursing, Rehabilitation or Assisted Living Facility is the easiest and best way to insure you and your loved ones are truly getting the compassionate and comprehensive healthcare you are entitled to.

Have the knowledge to know what should be provided, then get the experts’ insight into the care that is actually being provided.

Allow us to equip you with the information and understanding that will provide you with the sense of comfort and security you want.  This benefit will allow you to enjoy quality social time while having the confidence that the care being provided is being monitored by professionals.  Alastar also facilitates improved communication between the provider and the client by acting as a liaison when unexpected challenges arise.

There is uncertainty that occurs with management of sudden healthcare needs.  Alastar will be there to guide you through the process and set expectations of a stay at a Skilled Nursing/Rehabilitation Facility.  Our experience and guidelines for quality care in this industry affords us the knowledge to equip you with this valuable information.

Giving you peace of mind that allows you to focus on quality time with your loved one, rather than spending your precious time managing their care.

Regardless of the healthcare setting of the client, we accomplish this by several different interventions from our licensed professionals:

  •  Individual interview with resident
  • Interviewing  Care Associates
  • Visual assessment of the living environment
  • Visual assessment of social interactions
  • Medical Record Review

Using this information we compile a Comprehensive Status Report including Medical Record and Regulatory Interpretation as needed.
